It's not a cigarette holder. She's smoking from a Hookah. I've seen Ovie and Semin smoke that too.
But it does look like cigarette holder though. Hookahs are a lot bigger, just look at http://en.wikipedia.org/wiki/Hookah

Hookah is very tasty and it isn't too dangerous for the lung, as the smoke is being filtered by the water.

I'd rather Alex and Sasha had healthy, tobacco-free lives. I want them to be in good physical condition for a long time and tobacco use is one thing they have control over.
